The first four fields above are obviously pretty easy, but if you don't know the manufacturing plant where your truck was built or its build sequence, the Dakota VIN Decoder should be able to help you out. You may want to right click on the link to the VIN decoder and open it in a new window so you can refer to the info there as you fill out this form.

Ok, here's where you list the problems you've had with your truck that you'd like entered into the database. It would be really helpful if you could, for each problem, give me the approximate mileage on the truck when it first cropped up. Please note that if you don't list any problems, I can't add you to the database... The whole idea behind this is to track problems, not to keep a list of VINs... :-)
