2005 DML Calendar Info

I will need some basic info about you and your truck, to print in the calendar. I usually print a little blurb right on the photo itself with your truck, name, location and e-mail address. For example:


1996 Dakota Sport 5.2L V8 4x4

John Smith - Carlisle, PA - johnsmith@somewhere.com

As such, I need to collect this information from everybody. If you do not want your name, location or e-mail address printed in the calendar, simply indicate that in the appropriate place below. You can use the comments field to provide me with additional information, or ask questions, etc. If you have a preference as far as which month you would like your truck to appear in, please indicate it in the comments field. I can definitely make no guarantees that I will actually be able to comply with your request, but I will try.
