DML Calendar Order Form

Pricing (in US dollars) for the 2001 DML calendar is as follows:
The prices above include shipping to the US or Canada. If you would like your order to be shipped via priority mail (US only) instead of regular first class mail, please add $2.00 to your total to cover the additional postage. Canadian DMLers who would like their order to be sent via Global Priority Mail, please add $5 US to your total. If you are not in the US or Canada, or if you would like an even faster shipping method (next day, Express Mail, etc.) please contact me with the shipping method you'd like and I'll get back to you with a price.
Due to the economy of shipping more than one calendar at once, I was able to create a price break for ordering multiple calendars. Note: I am not making any money on these for myself; if I'm fortunate enough to come out of this in the black, all profits will go directly into the DML coffers in order to pay the list hosting costs, web site and domain fees, etc.

If you are planning to hang the calendar on the wall (as I immagine most people are), the calendars can be ordered with a 1/4" hole or a small "binder clip"; please specify which one you would like. (The clip will allow you to hang the calendar on the wall without having a hole through each picture, just in case you'd like to frame the pictures later, or something...) If you don't specify wether you'd like a hole, clip, or nothing, the calendar will come with the hole.

(The easiest thing to do would probably be to print this page and mail it along with your payment, but if that isn't possible, just be sure to include something telling me the quantity you want, the address to ship them to, and your e-mail address in case I need to get in touch with you.)

If you'd like to save time, don't feel like mailing in an order form, or would simply prefer to pay by credit card, I will gladly accept payment via PayPal. When sending the cash via PayPal, be sure to put a message in the "Note" field telling me the number of calendars you are ordering, the hanging method you'd like, and especially your mailing address.
